Hx: Reported severely constipated with possible ticks on front legs. Abd rads yesterday showed LARGE amount of feces in the colon, megacolon, and maggots on anus. Pt started on Miralax and warm water enemas.
S: Alert, mews, allows all handling
O: BAR-H, MMs pink and moist
EENT: No discharge OU, AU, nose – food caked onto nose.
H/L: Eupnic, not ausculted
Abd: Soft except for LARGE semifirm fecal mass in caudal abdomen. Nonpainful on palpation.
M/S/I: Amb x4. Feces leaking all over anus and perianal region.
UG: Female
Neuro: Alert and appropriate, no sign neurological deficiencies
A: Severely constipated with secondary diarrhea
Short-term prognosis: Good
P:
1. Continue SQF daily
2. Continue warm water enemas daily
3. Clean hind end daily
4. Recommend repeat rads in a few days, consider sedated de-obstipation if fecal mass is not getting smaller

Sedation – 0.05 mls of Dexdomitor and 0.05 mls of Butorphanol given IM. Reversed with 0.03 mls of Antisedan.

BCS = 2/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Deciduous
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, distended with severe impacted feces
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Diarrhea leaking from anus with mobile light colored worms
Assessment:
Severe constipation
Myiasis
Dehydration
Underweight
Prognosis: Fair to good
Plan:
AXR – confirms large amount of feces in colon, megacolon
SQF 15ml
Warm water enema – 2 ml
Bath to clean perineum of maggots, heat support
Keep in medical for treatment and monitoring
Continue warm water enemas SID
Miralax 1/8 tsp BID
Continue to monitor while at MACC
