Medical Summary 05/28/17 DVM Intake Exam Estimated age: 10 weeks. Microchip noted on Intake? No. History : Brought in by owner, no medical concerns noted. Subjective: Observed Behavior – Shy, hiding in back of carrier. Allows himself to removed with no problems, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=220, RR=24. BCS=5/9. ORAL: All deciduous teeth erupted, no oral lesions. EENT: OU – mod. mucopurulent ocular discharge. No nasal discharge, no sneezing. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic. ABD: Non painful, mildly pot-bellied, no masses/organomegaly palpated. U/G: Male, intact. MSI: Ambulatory x 4, no lameness. Full haircoat, no skin lesions. Debris, litter stuck to fur on tail and plantar hind feet. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Conjunctivitis, otherwise apparently healthy. P: Cleaned OU, applied Erythromycin ointment. Rx Erythromycin ointment OU BID x 10 days. OK for neuter surgery. Excellent prognosis.
