08/15/2017 Exam Type MS NEW URI – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is NONE, Weight 11.5 LBS.
Hx: Scrotal hematoma and wound on tail. Neutered on 8/8/17. Scrotal hematoma was painful to touch on 8/11/17, pt was switched to Clindamycin and warm compresses to hematoma. Simbadol extended through 8/13/17. S: Calm and quiet in kennel O: BAR-H, BCS 5/9, MMs pink and moist EENT: Serous nasal discharge, heavy salivation. No discharge OU. H/L: Sneezing. Not ausculted. Abd: Soft, no pain on palpation M/S/I: Amb x4. Scrotal hematoma is small, soft, dark/purple, no discharge. UG: Male neutered Neuro: Alert and appropriate, no sign neurological deficiencies A: 1. URI – possibly calicivirus with lingual ulcerations 2. Scrotal hematoma – resolving slowly Short-term prognosis: Good P: 1. Move to isolation, start doxycycline 1 ml PO SID x10 days 2. 08/03/17 17:40 Basic Information: Mr. Meesh is a one year old, male orange tabby cat. He was a gift from a friend at about a month and a half old. They cannot keep him anymore due to financial matters. He’s never been to the vet but has no known health problems. Socializaion: Around strangers Mr. Meesh is friendly and outgoing. He lived with children ages 12 and up and was gentle and playful around them and adults. He’s never been around other dogs or cats and has never bitten another human or animal before. Behavior: Mr. Meesh tends to scratch at the furniture around the home. He’s only been in the car twice and he meowed a lot. Starting about a month ago a few times this passed month he did start spraying around the home. He’s never had a bath or his nails trimmed but he loves being brushed. He isn’t bothered being picked up and gets a little afraid of the carrier and tenses up when putting him inside. For a New Family to Know: Mr. Meesh is friendly, affectionate and playful with a medium energy level. He loves to play with balls, strings and laser pointers. He’s only been an indoor cat. At night he sleeps in different areas in the home like the couch, the kitchen, wherever he feels comfortable that night. He mainly eats dry food. He’s mostly litter box trained and uses a hooded litter box with crystal litter. He doesn’t own a scratching post so he’ll use the side of the couch. Behavior During Intake: When I opened the carrier he popped his head out right away and willing to come out. I picked him up and helped him get the rest of his body out and collared him fine. I held him for just shy of a second and he didn’t mind; his body was very loose the whole time. He scanned negative for a microchip

Reaction when softly spoken to: Mr. Meesh looks at the assessor for a moment before starting to groom himself. Reaction to cage door opening: Mr. Meesh remains soft and relaxed. Reaction to touch: Mr. Meesh sniffs the assessors hand, accepts petting, and lifts his rear when pet on his lower back. He begins to purr and looks around while being pet. Reaction to being picked up: Mr. Meesh remains calm and allows handling. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Social, Calm, Curious POTENTIAL CHALLENGES: – Spraying – Mr. Meesh’s previous owner reported he has started to spray around the home.
