Mr Kitty is a six year old white neutered domestic shorthair. He was surrendered due to behaviors he was displaying in the home. He was given to owner by a local store. He was with his owner for the last three years. Mr Kitty has not been taken to the vet by owner.
Mr Kitty has lived with three adults in his previous home. Mr Kitty has been around children ages six, nine and one, owner stated he will swat and scratch the children if they try to engage him in play. Mr Kitty has not been around other cats or dogs. Mr Kitty often scratches in the home, owner stated he will swat when he does not want to be bothered. Mr Kitty has not bitten in his previous home.
Mr Kitty was not bathed or brushed. He will struggle when held and if placed in a carrier. Mr Kitty is described as independent and stay hidden in the home. Mr Kitty has been kept mostly indoors owner stated he would try to run out the home if the door is opened. Mr Kitty is litterbox trained and uses an open litterbox and clumping litter. Mr Kitty would scratch at that the fabric couch, he was not provided a scratching post.
Upon intake Mr Kitty had a tense body and shied away from counselor. He was receptive to petting, counselor was able to collar, scan (negative), and photograph.

EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: sl tartar
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: Not performed
Assessment – Patient appears to be in good health
