Montgomery A1122324 – 9 week male orange tabby:
DVM Intake Exam
Estimated age: 7-9weeks
Microchip noted on Intake? no
History : Unknown (no intake memo at time of exam)
Subjective: Kitten is BAR
Observed Behavior – Vocalizing (meowing) in carrier. Active and moving around for exam, but shows no signs of fear or aggression. Limited interaction due to Ringworm+
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
T = NA P = wnl R = wnl BCS 4/9
EENT: No globe OS, mucopurulent discharge and cloudy cornea OD; ears dark brown waxy debris, no nasal discharge noted
Oral Exam: Healthy deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Very distended. Non painful, no masses palpated
U/G: wnl
MSI: Ambulatory x 4, skin free of parasites, no masses noted, multiple patches of alopecia with crusting lesions on head, paws, trunk, and at base of tail (Woodslamp +).
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Ringworm+, Conjunctivitis (missing OS)
Plan: Itrafungol at 5mg/kg PO q24h x 21d with twice weekly lime sulfur rinses; first rinse done today after exam. Erythromycin ophth oint – thin strip OU q12h x 7-10d and Doxycycline 0.2mLs PO q24h x 10d.
Prognosis:
Fair-Good
