MONTANA was brought to the shelter with a mass like wound on his body. He also has diarrhea and is in need of dental care. MONTANA is not liking the shelter and would be better off in a home where he can get the follow up care he needs asap.

Observed Behavior – trying to lunge and bite, semi-feral
Has been observed to be eating and drinking
Evidence of Cruelty seen – NO
Evidence of Trauma seen – Yes

R = wnl
BCS 4/9
EENT: Eyes clear, no nasal or ocular discharge noted, brown debris in otic canals
Oral Exam: fractured right max k9 at distal crown, mod tarter max caudal teeth (age est 2-4 y)
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated, but gassy intestines and liquid diarrhea
U/G: fs
MSI: Ambulatory x 4, skin free of parasites, overall, healthy hair coat, caudal to left axilla, sq on ventral thorax 3 x 4 cm, irregular, necrotic, hemorrhagic, mass-like lesion, NOT associated w/ underlying muscle or body wall
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal:liquid diarrhea
Assessment: 2-4 yr fs dsh
1) Necrotic mass-like lesion – r/o penetrating injury (most likely) vs foreign body reaction w/ subsequent granuloma formation vs mass
2) fractured teeth and mod dental tarter
3) diarrhea – r/o endoparasites vs stress vs other infection (panleuk vs corona vs other)
4) ear debris – r/o debris vs mites
Prognosis:
1) necrotic lesion – excellent w/ removal, but would recommend biopsy to completely r/o possibility of mass
2) diarrhea – good to excellent
3) dental – good to excellent w/ full anesthetic dental treatment
4) ears – excellent
Plan:
1) clipped and cleaned/flushed wound under 0.03 cc dexdom/0.1 cc ketamine/0.08 cc torb IM
2) 0.4 cc convenia sq
3) 0.5 cc simbodol sq
4) 0.1 cc ivermectin sq
5) parvo snap – negative
6) recommending sx to remove necrotic tissue and presumptive granulation tissue, would biopsy to ensure not mass
