History : new intake on 4/22/17 Subjective: Observed Behavior – eating in cage before exam, hissed and backed up with ears back when attempted to touch Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective — limited exam due to behavior T = — P = WNL R = WNL BCS 4/9 EENT: eyes clear OU, audible nasal congestion, sneezed on exam Oral Exam: observed when hissing – missing many teeth, halitosis, severe periodontal disease PLN: not assessed H/L: not assessed ABD: not assessed U/G: not assessed MSI: Ambulatory x 4, no obvious lesions, left ear tip (also has old wound to right ear tip) CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not assessed Assessment ~8 year old MN URI severe periodontal disease (recommend dental with extractions after placement) Plan Doxycycline (50 mg/ml): 0.75 ml PO SID x 10 days (recheck on day 5 and 10) Prognosis: Good SURGERY: already neutered
