Oral Exam: 6w age based on dentitia
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, sl wheezing, dyspneic, no murmur, 5% dehydrated, air passing through both nostrils on slide test
ABD: Non painful, no masses palpated
U/G: fi
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: n
Assessment: oen-mouth breathing, bradycardi, hypothermic, mild dehydration
r/o pneumonia vs uri vs underlying cardiac
Prognosis: guarded currently pending dx
Plan:x-rays to r/o pneumonia vs cardiac
ivc and LRS @ 3ml/hr, pcv/ts, ampicillin 17 mg iv tid and nebulize bid
monitor temp and HR
check pulse-ox if possible
SURGERY:
Temporary waiver due to underweight and current condition
DVM exam 12/17/18
Hx: presented with open mouth breathing, radiographs showed diffuse bronchiole pattern with possible pleural fissue line; started on IVF and ampicillin
S/O
-QAR, docile but interactive, allows all handling
-mm lgt pk, tacky
-in cage with littermate, canned food appears eaten but unsure if molly is eating
-no nasal discharge or congestion
-OU: mild blepharospasm
-mild inspiratory dyspnea, decreased lung sounds dorsally, mild crackles ventrally
-soft abdomen
A
1. Dyspnea, improving, suspect pneumonia; r/o CHF vs other infectious
2. Dehydration, improving
P
-continue IVF at 3 ml/hr
-cerenia 0.15 ml IV SID x 3 days
-continue nebulization, ampicillin
-recheck radiographs tomorrow
-prognosis: guarded to fair; suspect pneumonia; overall condition appears improved today
