MEDICAL:

Area of alopecia on digit noted on rounds
S/O: BARH. Very nervous, tense, hissing, retreating, tries to flee, allowed all handling with struggle once gently restrained
EENT: Eyes clear, very mild serous nasal discharge
HL: Sneezed 1-2 times
INTEG: Alopecia with flaky dermatitis encompassing entire digit of LF digit 5, ~1.5cm area of alopecia with flaky dermatitis at right lateral thigh
MS: Ambulatory x 4
UG: Female
Woods lamp positive at LF digit, right lateral thigh and right inner base of pinnae
A: Ringworm+, mild URI
P: Rec doxycycline 0.25ml PO q24 x 10 days, lime sulfur dip q72hrs, terbinifine 1ml PO q24 x 21 days
DTM culture to be taken prior to lime dip. Good prognosis with appropriate treatment and management

H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment – intact female
Plan – OHE
Prognosis: good
SURGERY: Okay for surgery
