DVM Intake Exam

Microchip noted on Intake? negative
History : stray, reported to be injured. Given simbadol overnight.
Subjective: BARH. Eating well.
Observed Behavior – Tense, nervous, and painful. Tried to bite several times during exam but also is painful so difficult to fully assess.
Evidence of Cruelty seen – no
Evidence of Trauma seen – yes
Objective
P = wnl
R = eupneic
BCS 5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: clean adult dentition, no oral lesions noted, mild abrasion to lower lip/lip avulsion
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: FI, no MGTs, no vulvar d/c
MSI: Non-ambulatory, cannot use front legs and resents PE, HL appear wnl, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment:
Suspect high rise
Right complete closed transverse fracture of the distal radial diaphysis
Left complete closed transverse fracture of the distal humeral diaphysis with significant displacement
Greenstick fracture of the left mid-radial diaphysis
Slight lip avulsion
Plan: Continue to monitor while at BACC
Sedated with telazol 0.1ml IM for rads
Forelimb radiographs-Right complete closed transverse fracture of the distal radial diaphysis, Left complete closed transverse fracture of the distal humeral diaphysis with significant displacement, Greenstick fracture of the left mid-radial diaphysis, mild SQ emphysema likely from MC, thorax appears wnl, abdomen appears wnl
Recheck daily
Start simbadol 0.24mg/kg SQ SID x5d until 1/22
Place spoon splint of right FL-stirrups, cotton padding, caudal spoon splint, cotton padding, vetcling, vetwrap
Rec surgical repair of FLs
Start onsior 2mg/kg SQ SID x3d until 1/19
Prognosis: Good
SURGERY: temporary waiver due to injury
