S/O: Cat is QAR. Thin body condition. Multiple scabbed wounds with purulent and bloody discharge affecting both ears and peri-ocular area. There is a deep laceration extending from upper eyelid OD caudally. Able to visualize cornea OU, appears clear. Globe OS appears intact, but unable to visualize cornea. Upper eyelid defect OS.
A: Wounds to ears and peri-ocular area. Rule-out self inflicted trauma due to ear mites vs other
P: Recommend New Hope placement ASAP. Cat will need sedation and/or anesthesia to debride wounds. Applied Revolution to treat for ear mites. Applied Terramycin OU – continue thin strip OU q12h.
PROGNOSIS: Fair-Good with appropriate wound care

EENT: Left eye clear, lid defect with periocular dermatitis, unable to visualize right eye due to swelling and surrounding abscess, moderate amount of dark brown flaky debris AU- scabby, disfigured and ulcerated pinnae, no nasal discharge noted
Oral Exam: Teeth clean, pink mm, adult dentition fully erupted, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, small healing wound at left medial carpus; abscess at top right of head extending to right eye and reaching towards left ear, both ears disfigured with scabby dermatitis, very delicate tissue that sloughs and tears with any manipulation
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: 6month old kitten with significant wound/abscess to face and head, may be self inflicted from ear mite infestation
Plan: Rec revolution, clean ears, simbadol 0.21ml SQ q24 x 3 days, clavamox 0.4ml PO q12 x 14 days, LRS 50ml SQ q24 x 3 days. Rec placement ASAP for wound care – will require special bandage to promote healthy granulation tissue and healing following cleaning and debriding of wounds
Prognosis: Good
SURGERY: Temporary waiver due to wounds
