Subjective: BAR. ~5% dehydrated. No csvd.
Observed Behavior – A little timid, but allowed petting. Rest of exam was sedated for wound care.
Evidence of Cruelty seen – no
Evidence of Trauma seen – yes, right tibia is broken
Objective
T = 97.1F (post sedation and wound care)
P = 180bpm
R = 30bpm
BCS 4/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: pale pink MM, sl tacky, clean adult dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact female, no discharge
MSI: Ambulatory x 3 with NWB right hindlimb lameness; palpable right tibial fracture- ~1cm in diameter deep wound noted on the medial aspect of the right antebrachium with exposed bone noted; skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
Two view pelvic and right hindlimb radiographs: complete, severely displaced, oblique fracture of the right tibia and fibula noted
CBC: Mild, non-regenerative anemia (Hct 30.1, Retic 3.6)
Chemistry: hyperphosphatemia (8.4)
Assessment
1) Open right tibia/fibular fracture
2) Mild, non-regenerative anemia- given #1, suspect acute hemorrhage from fracture site vs anemia secondary to chronic inflammation vs less likely neoplasia
3) Hyperphosphatemia- suspect secondary to age vs artifact (hemolysis) vs severe skeletal muscle injury vs less likely renal disease
Prognosis: fair
Plan:
– Sedated with Telazol 0.05ml IM then maintained on isoflourane
– The hair around the wound was clipped and the skin scrubbed with dilute chlorhexidine solution and saline. The wound was then thoroughly flushed with multiple rounds of sterile saline. A wet to dry bandage was then placed.
– Recovered uneventfully from sedation
– Monitor for worsening signs of anemia (lethargy, tachycardia, bounding pulses)
– Seek NH placement – needs RHL amputation
Treatments
Given LRS 100mls SQ once
Onsior 2 mg/kg SID x 3 days
Simbadol 0.24mg/kg SID x 7 days then re-eval
Enrofloxacin 5 mg/kg IM SID x 10 days then re-eval
