Medical Summary 02/25/16 16:37 INITIAL EXAM approx 4-5 month old female intact DSH cat BAR, hydrated, BCS 3/9 (slightly underweight) No URI signs, eyes and nose clean moderate amount of dark brown debris AU – ear mites mm’s pink and moist, adult canine teeth coming in amb x 4, no lameness or neuro deficits noted heart and lungs WNL on auscultation abdomen soft and non-painful no alopecia, no fleas or flea dirt seen PLAN gave rabies vaccine SQ over right hind leg gave FVRCP vaccine SQ over right front leg applied activyl topically gave 0.4 ml pyrantel PO sign up for panacur and ponazuril deworming sign up for tresaderm AU BID x 10 days excellent prognosis
