Medical Summary 11/17/16 Sex: Male intact Scan: Negative Estimated age: 1-2 years Fleas: Lots of flea dirt. Hx: Was found yesterday, reportedly wasn’t breathing well last night S: Meows, calm, allows handling O: BAR, BCS 1-2/9, good skin turgor, estimated 5% dehydrated, MMs white and tacky EENT: Moderate thick purulent ocular discharge OU. No discharge AU, nose. Clean teeth. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Thickened and ropey small intestines. No pain on palpation, no masses palpated M/S/I: Amb x4. Soft tan feces all over pt’s feet and body. Dirty fur coat. Alopecia (quiet skin) at base of ears. UG: Male intact, testicles soft and symmetrical. Parvo test: Negative A: 1. Emaciated, thickened intestines – R/O GI parasites, retroviral infection, other 2. Purulent ocular discharge – R/O chronic conjunctivitis vs. other Short-term prognosis: Fair Current surgery status: Temporary waiver for emaciation P: 1. Administered Activyl for cats 2-9 lbs and pyrantel 0.6 ml PO during exam 2. Monitor weight 3. FeLV/FIV test 4. Monitor appetite, diarrhea, vomiting 1088
