Medical Summary DVM Intake Exam Estimated age: Microchip noted on Intake? neg History : o surrender, previous o states p has had a neuro issue Subjective: Observed Behavior – allows all handling no aggression Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 180 R = 36 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, no palpable spay scar, no visible tatoo MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, no palpble ortho instabilities or pain on HLs, no lesions CNS: mentation appropriate, PLR, menace intact OU, no obvious intention tremors in the head but when walks is unsteady and at times wide stanced in HLs, also unsteady as takes steps, but is ambulatory with no limp. No spinal pai Assessment r/o cerebellar hypoplasia, other (toxin, other) Plan ok for spay contacted NH Prognosis: good with an owner who is understanding of a CH cat’s limitations, can lead a healthy life with good quality of life SURGERY: Okay for surgery
