SAFE 11/24/17 – Poor Meadow was found in the engine of a car in finders brother’s auto shop. Meadow is an approximately 3 month old female domestic short hair kitten that presented to MACC with two severe wounds on the left hind limb as well as healing wounds and residual swelling along the right ear base that are consistent with bite wounds. Based on the level of tissue necrosis as well as the overall appearance, the wounds are suspected to have occurred 7-10 days prior to presentation. MEADOW needs medical attention and out of the shelter.

serosanguinous strike through all layers of bandage this afternoon; severe wounds appear improved today with minimal persistent necrosis; exposed muscle tissue appears viable; sutures intact in proximal aspect of main wound along LH
ambulatory with significant weight-bearing paresis/knuckling of LH; hyperextension of tarsus
replaced bandage, change in 24-48 hours
d/c hydromorphone and gabapentin, start simbadol this evening
prognosis: good overall, will likely require rehabilitation of LH due to suspected nerve damage and tarsal laxity but suspect can have a good quality of life in the proper home

-AD: ear base sits lower than AS and is freely moveable; superficial ulcerated wound with mild dermal necrosis on back of pinna; irregular pinnal margins at tip and base due to previous trauma with mild crusting; overall appropriate tissue healing; edematous swelling at ear base improved-resolved

Meadow is an approximately 3 month old female domestic short hair kitten that presented to MACC with two severe wounds on the left hind limb as well as healing wounds and residual swelling along the right ear base that are consistent with bite wounds. Based on the level of tissue necrosis as well as the overall appearance, the wounds are suspected to have occurred 7-10 days prior to presentation. At this time, she is mentally and neurologically appropriate with no concerns of Rabies. Please feel free to contact me directly with any questions or concerns you have regarding Meadow.

Severe wounds to distal LH noted on intake but difficult to assess full extent; was given 0.1 ml hydromorphone and convenia injection on intake
S/O
-BAR, affectionate and interactive, allows all handling
-great appetite, normal bowel movement in box
-mm lgt pk, sl tacky; deciduous dentitia
-no nasal discharge or sneezing
-mild to moderate diffuse nonpainful, soft swelling along R ear base and side of face
-AD: fibrosis of tip of pinna with healing wounds along edges; area of ulceration on ear pinna suspected to be healing wound
-eupnic, purring loudly on auscultation
-soft abdomen
-LH: non weight bearing lameness, voluntary motor movement present, withdrawal decreased but intact (likely due to pain); mild tarsal hyperextension; severe wound distal to stifle with exposure of underlying musculature with small area of exposed fibula; necrotic tissue and debris along wound, appears 7-10 days old; smaller laceration over dorsal aspect of metatarsals with tendon exposure and mild necrosis along edges of wound

-sedated with 0.05 ml hydromorphone/0.1 ml ketamine/0.1 ml dexdomitor
-debrided necrotic tissue along wounds, flushed with sterile LRS and then dilute nolvasan
-partially closed proximal aspect of larger wound
-placed TAB ointment and telfa pad along wounds and placed large bandage over limb
-gave 30 ml LRS SQ
-gave onsior 0.1 ml SQ SID x 3 days
-baytril 22.7 mg/ml: gave 0.46 ml IM, start PO tomorrow
-gabapentin 50 mg/ml: 0.2 ml PO BID
-change bandage in 2 days

Subjective: BAR, very sweet and friendly, purring, no signs of aggression. Very hungry and ate throughout the PE

EENT: Eyes clear, ears mild debris, no nasal or ocular discharge noted. Right ear sits lower then the left, base of the ear more moveable compared to the left, abrasions on the external pinna.
Oral Exam: neg oral
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Ropey intestines, Non painful, no masses palpated
U/G: FI
MSI: Ambulatory x 3. Trauma to LHL, fur has dried blood making it difficult to fully examine, visible tendon and possibly bone on lateral aspect of femur and hock. Tarsal laxity and negative deep pain response. Skin free of parasites, no masses noted, dirty unkempt hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
Assessment : approx. 3m FI DSH stray
Right auricular trauma
LHL trauma with exposed tendons +/- bone
Prognosis: guarded to poor
Plan:
gave convenia 8mg/kg SQ
gave hydromorphone 0.1mg/kg SQ. continue q8hrs. Wanted to give simbadol but not available. Regular buprenorphine also not available.
LRS 22mL SQ once. Continue PRN
Tomorrow recommend :
– sedation
– clip/clean and examination LHL wound
– LHL rads
– skull rads to assess right auricular trauma
– p likely will need LHL amputation
– Humane euth if poor prognosis
