Medical Summary DVM Intake Exam Findings History Subjective Observed Behavior – very hungry, allows handling, especially when eating Evidence of Cruelty seen – underweight- memo states found in home with other underweight cats, some were dead, o was taken to hospital for possible illness Evidence of Trauma seen – no Objective T = not taken purring, but eupneic, unable to ascult cardiac sounds, unable to obtain RR and HR, BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: pink gums, approx 3% dehydration, slow skin tent PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, grossly nromal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: thin/underweight, mild dehydration Fast Track? no Plan wait until gains weight to spay, temporary waiver until then LRS 50ml SQ SID x4 days, first dose given at intake prognosis: good
