DVM Intake Exam Estimated age: 5-7 years Microchip noted on Intake? negative History : stray Subjective: BARH Observed Behavior – very sweet but nervous. Easily handleable. Did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: adult dentition, mild dental staining, no oral lesions noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI, no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat except for very mild thinning of hair on tail CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy Mild alopecia on hair r/o FAD vs other allergies Plan: Continue to monitor while at BACC Prognosis: Excellent SURGERY: Okay for surgery
Pre-OHE exam: 1431 BAR, hydrated EENT: clear au/ou PLN: wnl H/L: 120 regular, no murmurs, ssp. Resps are clear and eupnic, no signs of ocular or nasal discharge. U/G: female intact ABD: benign MSI: ambulatory x 4, in good flesh, good coat Neuro: nsf A: in apparent good health P: okay for surgery
New URI noted on rounds History : stray intake 10/1-mild alopecia but otherwise apparently healthy Subjective: QAR. Congested with ocular and nasal d/c. Decreased appetite Objective EENT: mucoid ocular d/c ou, serous nasal d/c bilaterally ORAL: drooling H/L: sneezing and congested Assessment: URI with conjunctivitis Hx Mild alopecia on hair r/o FAD vs other allergies Plan: Continue to monitor while at BACC Move to iso Start doxycycline 10mg/kg PO SID x10d until 10/18 Start erythromycin OU BID x10d until 10/18 Recheck day 5, 10 Consider app stimulant if still not eating with treatment Prognosis: Good
