EENT: nuclear sclerosis, ears clean, no nasal discharge noted
Oral Exam: Missing most teeth, severe tartar of remaining teeth, pink mm, no oral ulcers, open wound vs mass at left side of mouth caudal to commissioner of lip, bleeding when manipulated, appearance of soft gingival hyperplasia, firm swelling caudal to wound at ramus of mandible
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty unkempt hair coat with large arears of alopecia with no underlying dermatitis, flea dirt
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Geriatric, dental disease, underweight, dehydrated, unkempt, alopecia, oral mass – neoplasia (SCC vs other) vs extension of severe dental disease vs other
Plan: Convenia 0.26ml SQ given, Simbadol 0.33ml SQ. Blood work pending. Continue to monitor while at BACC
Prognosis: Poor
