Medical Summary DVM Intake Exam Estimated age: ~10+yrs, male Microchip noted on Intake? No History : Sick stray cat Subjective: Observed Behavior – Dull/QAR. Allowed all handling. Excellent appetite but very weak, severely dehydrated Evidence of Cruelty seen – no Evidence of Trauma seen – yes Objective P = 132 R = increased respiratory effort BCS 2/9 EENT: Large corneal scar with synechia OS, sunken eyes, ears clean, no nasal discharge noted Oral Exam: Dental disease, pale mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, infected wound down to muscle layer at lateral right front limb with necrotic tissue extending proximally to scapula CNS: mentation appropriate – no signs of neurologic abnormalities CT light positive for FIV Assessment: Geriatric, significant wound, emaciated, dehydrated, FIV+ Plan: Rec LRS 100ml SQ q24 x 3 days, baytril 0.6ml SQ q24 x 7 days and augmentin 0.9ml PO q12 x 14 days (gave first dose of all). Will require surgical repair of wound Prognosis: Poor SURGERY: Permanent waiver due to age and medical condition
