Medical Summary Microchip: negative Sex: intact male Age: appx 5y Mentation: QARH Eyes: heavy tear staining and some purulent d/c OU, possibly conjunctivitis but no chemosis Ears: clean Nose: no d/c Teeth: mild staining with heavy drooling (reported vomiting when found and possibly nauseous on exam, gagging at pyrantel) If abnormal BCS: WNL Skin: WNL Hair Coat: alopecia around neck, possible flea dermatitis but no flea dirt seen and no current wounds (some old scabs) Declawed: no Any injuries: no; reported hx diarrhea and vomit prior to intake, some beige solid feces seen on exam (malabsorption?), parvo SNAP (-) Behavior: allowed all handling but nervous and tense (could also be from illness) Medication: N/A, preventatives given but dewormer withheld
