Medical Summary DVM Intake Exam Estimated age: 1yr Microchip noted on Intake? none scanned History : Subjective: Observed Behavior – at first quiet then became nervous and tense, hissing and growling- r/o scared from transport Evidence of Cruelty seen – no Evidence of Trauma seen – no LIMITED EXAM d/t temperment at intake: Objective unable to take TPR BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: used tongue depressor- pink gums, mild gingivitis PLN: unable to examine H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated- brief palpation U/G: FI grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment apparently healthy from brief exam Plan ok for spay Prognosis: good SURGERY: Okay for surgery
