Medical Summary DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? neg History : Found in neighborhood on her way to work – 2 kittens together (A1115275 littermate) Subjective: BARH. MPOD. Dry nasal d/c r nostril. Observed Behavior – Easily handleable and afffectionate. Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = R = BCS = 4/9 EENT: Eyes MPOD, mod conj erythema. Ears clean. Dry nasal d/c R nostril. Oral Exam: normal kitten dentition PLN: No enlargements noted H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. Pot bellied. U/G: Female MSI: Ambulatory x 4, fleas, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: n Assessment Conjunctivitis -moderate. URI Both littermates placed as URI. Plan: Terramycin eye oint OU BID x 7 days. Clavamox 0.15 cc PO BID x 10 days. Prognosis: Good SURGERY: Temporary waiver due to underage and conjunctivitis/ URI.
