11/18/16 20:17 Socialization Around strangers Lyger is friendly andoutgoing. He has been around another small breed dog and got along great and is not shy around cats whom he also shared a home with. He is exuberant, affectionate & playful with the adults. Lyger hasn’t bitten/scratched any person or animal. Behavior During car rides Lyger will meow and or be relaxed. He isn’t bothered by the following: being picked up/held, being brushed or with being placed into a carrier. Lyger hasn’t been given a bath, had his nails trimmed or been disturbed while sleeping. New Family Lyger is described as friendly and affectionate with a low-high activity level. When the owner is home Lyger follows them around. He enjoys string toyssometimes and is an indoor only cat that sleeps around the home. Lyger eats dry Purina Cat Chow and 9 lives once to twice a day. He has an uncovered litter box trained with clumping litter within which he doesn’t always use recently. Lyger does not scratch much but sometimes on furniture. Intake Lyger did not stand up during intake and had a hard time standing but allowed all handling.

Reaction to assessor: Lyger immediately comes soliciting at the front of the cage. Reaction to door opening: Lyger remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Lyger head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: Allows the pickup and remains calm. Placement determination: Beginner Lyger interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

S/O: very friendly, docile cat reported to have signs of uri and not be eating at home, reported to be eating here at acc ABN: SNP, non painful, some mild possibly fecal perianal staining EENT: pink gums, mild tartar and gingivitis, no nasal or ocular d/c, no sneezing H/L: purring, but eupneic, unable to ascult cardiac sounds, RR, or RE M/S: amb x4, bCS 3/5 with some epaxial mm wasting A: geriatric possible D reported to have been sneezing and sounding congested- not observed gave food- ATE WELL, urine in cage P: CBC/chem in house-GLU=457mg/dl, CREAT- mild dec 0.7 mg/dL, TBIL 1.0mg/dL-DIABETIC started on Vetsulin (u-40) 1unit SQ after eating 11:20AM—gave with no problems taking the injection- very docile check for D, if present will start metronidazole monitor appetite check BG tonight at 8PM- if >400 and eating can give one unit of Vetsulin (u-40) SQ; IF NOT SURE DO NOT GIVE- then for DVM to decide to give insulin in AM and check BG check app contacted NH for possible placement prognosis: fair- as a diabetic, will need daily insulin, make sure p is eating, and regular monitoring of the diabetes. Strongly rec UA to check for glucosuria- confirmation of DM and to monitor regulation. Good news p is eating and BAR. Rec BG curve or fructosamine, UA with c/s as diabetics are suspectible to UTI’s, also rec diabetic iet like Purina DM
