YES; 985112003906023 History : Relinquished for owner allergies Subjective: BAR, sweet, somewhat nervous Observed Behavior – allows all medical handling, nervous Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 180 R = eup BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gumline hyperemia, mild dental calc PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no abdominal masses, 2×2 soft swelling that palpated connected to body wall possibly from spay site – not reducable U/G: SF, spay scar apparent but no tattoo MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 5 year old SF DSH 1. SQ swelling abdomen r/o spay hernia v lipoma v other neoplasia 2. Otherwise healthy Plan -Apparently healthy adult -Consider work up for abdominal swelling (consider explore +/- rads +/- aspirate -Proceed with vax, activyl, pyrantel -Retro negative Prognosis: Good SURGERY: Already spayed
