Medical Summary 02/23/16 16:39 Scan : Sex: Spayed, left ear tip Approx 8-10yr S/O: QAR. Nasal discharge, OS crusted shut. Attempted to flee during exam. Sedated w/ 0.15ml Telazol IM to faciliate complete exam w/ minimal stress. FLushed OS, chemosis, conjunctivitis. Iris atrophy OU. Lingual ulcers on lateral aspect of tongue. Missing all teeth except upper canines, and right lower premolar. PM has resorptive lesions. Moderate dark debris AU, cleaned ears. Left ear tip. Grade 3 Right systolic basilar murmur. Normal BV sounds, referred upper airway noises. Trimmed nails A: URI, conjunctivitis, FIV, heart murmur, otitis P: Administered 0.36ml Convenia SQ, 0.15ml Buprenorhine SR. Start erythromycin OS BID, and doxycycline 0.8ml PO SID, behavior permitting treatments. PROGNOSIS: Fair short term pending response to treatment.
