SAFE 4/12/17 *BONDED WITH PRINCE BOY – MUST BE PLACED TOGETHER* Little Bee was dumped for allergies – however this neutered boy also has some allergies of his own and some dental disease which causes him to drool.. He also has a heart murmur and congenital defect where he has deformed/very short front legs but can put weight on them. He is also polydactly on his hind legs. Cat was brought in with housemate.

Medical Behavior Evaluation GREEN
Medical Summary History Owner surrender with 1 other cat. Born with deformed front legs. Subjective Purring, allows most handling but moved away very quickly when hind end was touched Objective BCS 6/9, MMs pink EENT: Mild crusts at medial canthus OU. Clean ears. Oral Exam: Severe gingivitis and tartar, drooling heavily. PLN: No enlargements noted H/L: Grade 2-3/6 systolic heart murmur, PMI left side. NSR. CRT < 2, Lungs clear, eupnic ABD: Slightly tense, no masses palpated U/G: Male neutered MSI: Front legs have abnormally short humerus and radius/ulna, with contracted joints. No fractures, swelling or wounds noted. Pt seems to be able to bear weight well on both front legs. Large amount of flea dirt. Large area of alopecia, erythematous skin and large crusts at tailhead and to right of tail, and on the caudal aspect of the left hind leg. Pruritic. Polydactyl – 6 toes on both hind feet. Large amount of flea dirt. CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment 1. Heart murmur – R/O congenital disease vs. other cause 2. Congenital deformity of front legs 3. Dermatitis – R/O flea allergy dermatitis vs. other cause 4. Severe dental disease 5. Drooling – R/O oral pain vs. other 6. Polydactyly Plan 1. Benadryl 12.5 mg PO BID x15 days 2. Simbadol 0.7 ml SQ SID x7 days 3. Recommend echocardiogram and dental cleaning within a few weeks of placement 4. Amoxiclav liquid 1.6 ml PO BID x15 days 1088
