Medical Summary DVM Intake Exam Estimated age: 4-5 weeks based on dentition and weight Microchip noted on Intake? Scanned neg – MC placed History : Kitten found outside Subjective: Meows, struggles when restrained. Allows handling. Objective BAR-H, MMs pink, BCS 4/9 EENT: Moderate mucopurulent ocular discharge, mild serous nasal discharge. Clean ears. Oral Exam: Deciduous teeth present PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Soft, non painful, no masses palpated U/G: Female kitten, presumed intact MSI: Ambulatory x 4, skin free of parasites, no masses noted. Diarrhea on hind legs. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Diarrhea – R/O parasites vs. bacterial enterocolitis vs. other 2. Conjunctivitis and mild URI Prognosis: Good Plan 1. Doxycycline 0.1 ml PO SID x10 days 2. Erythromycin OU BID x10 days 3. Ponazuril 0.25 ml PO SID x5 days 1088
