Medical Summary DVM Intake Exam Estimated age: 4 yrs Microchip noted on Intake? yes (981020019680158) History : O/S due to moving to place that does not allow pets. Subjective: Cat is BARH Observed Behavior – Cat readily approaches humans at front of cage, appears relaxed, eating, rolls and solicits attention. Upon taking cat out of cage he began with some mild hissing, piloerection, and excessive sniffing when being weighed. Cat began paw lifting on examination of head and quickly escalated to swatting with nails extended. Unable to handle safely; had to place towel over cat to get back in cage. Behavior seems to stem from overstimulation more than true aggression; needs some time to adjust. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = NA R = NA BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Grade 0-1/4 dental dz PLN: NE H/L: NE ABD: NE U/G: appears wnl MSI: Ambulatory x 4, appears to have healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Acceptable candidate for adoption or rescue pending behavior re-evaluation Prognosis: Good-Excellent SURGERY: Already neutered
