SAFE 4/19/18 Mama and her two kittens are looking for a new home @MACC. Leonita is a 2 year old black and white female DMH. She was bought from a pet store when she was 1 month old. Her previous owner surrendered her because they had too many pets.

Your newly adopted pet has been diagnosed with A HEART MURMUR and the staff veterinarians are issuing a permanent waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment. Your veterinarian will advise you if surgical sterilization is appropriate.

Observed Behavior -hisses and regresses when approached; allows gentle handling for exam but growls through entire exam

Evidence of Trauma seen -n

EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam:mm pk, moist; CRT <2 sec; minimal tartar/staining
PLN: No enlargements noted
H/L: eupnic, clear lung sounds; grade II-III heart murmur
ABD: Non painful, no masses palpated
U/G: female intact
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal:grossly normal

Assessment
1. Post partum
2. Heart murmur-r/o secondary to stress vs benign vs primary heart disease

Prognosis:fair to good

Plan:
-recommend echocardiogram if heart murmur persists

SURGERY:permanent waiver due to heart murmur

Around strangers, Leonita is friendly and outgoing and exuberant at play with adults.
How is this cat around children?
Leonita has spent time with children age 4 and 7. With them she was relaxed and affectionate and exuberant at play.
How is this cat around other cats?
Leonita lived with 6 other cats, 2 adult males and of her 4 kittens. She was relaxed and affectionate with all of them. When she played she was exuberant.
How is this cat around dogs?
Leonita has not been around dogs.
Behavior Notes
Leonita is relaxed during car rides. She enjoys baths, having her coat brushed and being picked up. She isn’t bothered if she is placed in a carrier or disturbed while sleeping.

Leonita has never attempted to bite a person or animal.

For a New Family to Know
Leonita is described as friendly, affectionate, playful, confident and quiet. When her previous owners were home she followed them around. She likes to play with anything. She was an indoor/ outdoor ct and had free choice. She usually stayed in the backyard and didn’t wander far from the home. Since she had her babies she has not left the home. At night she slept in her owners bed. She ate dry Friskies. She is litter box trained and used a hooded litter box and clumping litter. She had a vertical scratching post made of rough rope.

Behavior Notes:

Leonita had a relaexed body and allowed me to pet her and pick her up. she head butted as I pet her and purred. She allowed me to interact with her babies and collar her.
