Medical Summary DVM Intake Exam Estimated age: 6 months Microchip noted on Intake? none History : owner surrender due to cost of veterinary care Subjective: QAR Observed Behavior – very sweet but shy; easy to handle and examine Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not taken P =WNL R =WNL BCS 4/9 EENT: crusting grey discharge AD, not pruritic, canals do not appear irritated Oral Exam: mm pk, moist; CRT <2 sec; all adult teeth present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: ambulatory x 4 with no lameness noted diffuse moth eaten alopecia-woods lamp positive on all lesions on nose, face, digits, dorsum CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. diffuse ringworm Plan terbinafine (3.5 mL PO SID of liquid or 1/4 of 250 mg tablet) x 4-6 weeks until DTM culture neg lime sulfur dips twice weekly until clinical resolution needs foster/NH placement Prognosis: good with treatment SURGERY: temp waiver due to ringworm
