Subjective: patient QAR. eyes and ears closed. Umbilical cord dry and attached. Patient was not suckling and will not take to bottle. Allowed to syringe feed.
Observed Behavior – allowed all handling
Evidence of Cruelty seen –

Objective
P = 200 plus R = 34 BCS4/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: no cleft palate
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment – neonate
Plan – new hope palcement
Prognosis: good-fair
