Crouched in back of kennel, ears forward and eyes wide. She tolerates petting initially, turning her head to watch my hand, then leans in and steps forward to greet me with a raised tail. Sweet and social, doing well after first day! Allows all petting.

History: Stray cat. Noted to be emaciated and have a heart murmur on intake.
Subjective: BAR. ~5% dehydrated. Smeared feces found on towel. No vomit noted. Ate tuna last night.
Objective
P = 200bpm
R = wnl
BCS 2/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: mild to moderate dental/periodontal disease
PLN: No enlargements noted
H/L: Grade V/VI heart murmur, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: female, no discharge
MSI: Ambulatory x 4, moderate to severe diffuse muscle wasting/cachexia, skin free of parasites, no masses noted, moderately roughened hair coat, ~1cm in diameter scabs noted on both hocks
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
Assessment
1) Heart Murmur (V/VI)- r/o top differential would be HCM (primary) vs less likely physiologic
2) Emaciation/Cachexia- Given unremarkable bloodwork and urine, top differentials are chronic GI disease (IBD/lymphoma vs less likely pancreatitis) vs systemic neoplasia
Prognosis: guarded
Plan:
Cerenia 1 mg/kg SQ q24hr x 2 days
Metronidazole 10mg/kg PO q12hr x 7 days
Recheck appetite/weight tomorrow
Long term recommendation would be echocardiogram and abdominal ultrasound to better assess for underlying conditions

Korin was noted to have vomited in the cage once.
Administered LRS 40mls (20ml/kg) SQ once and Cerenia 1 mg/kg IV once
Bloodwork:
CBC- Leukocytosis (22K) with neutrophilia (19K), PLT 73K (likely due to clumping)—> top differential given age and species of patient would be stress leukogram vs chronic primary GI disease (IBD/intestinal lymphoma), inflammatory disease (pancreatitis) vs systemic neoplasia vs less likely infectious
Chem- BUN 37 (mild elevation), Creat (0.8)- due to muscle wasting, this could be falsely low, recommend SDMA/UA to further evaluate for renal disease
mild hyperglobuinemia (5.3)
Interpretation: very mild azotemia – r/o pre-renal (dehydration) vs renal (chronic renal disease vs acute on chronic vs renal neoplasia ) vs post-renal (ureteroliths).
Recommend SDMA and UA to further evaluate for renal disease
Recommend abdominal ultrasound to further evaluate for neoplasia, chronic GI disease.
Recommend echocardiogram to evaluate for underlying heart disease

Prognosis: fair to guarded depending on underlying cause
Plan: As patient is stable, no treatment currently needed unless patient is noted to anorexic, vomiting, etc.
Consider full bloodwork (CBC/Chem/T4) if clinical signs noted. Long term recommendation would be echocardiogram and abdominal ultrasound to better assess for underlying conditions
