Subjective: Stray. Visible classic ringworm lesions on forehead and nose. Behind the hind paw pads crusty with either lesion or bacterial growth. Limited fluorescence under Wood’s Lamp. Vet 1524 confirms probable ringworm; DTM taken
Eyes: Both eyes crusty, clear fluid
Ears: Oily; cleaned
Oral Exam: nose/throat clear, teeth white, gums pink. Vet 1524 confirms conjunctivitis; eryc ointment begun.
Heart: wnl
Lungs: wnl
Abdomen: mildly enlarged “worm belly”
Musculoskeletal: no visible injuries
Mentation: appropriate
Preliminary Assessment: Ringworm, conjunctivitis in underage kitten
Plan: Foster/NH placement.
