I came in the shelter as a STRAY on 07/12/2016 from NY 10025, owner surrender reason stated was OWNER DIED.

7/21/16 Pt resting in cage, alert, compliant for exam Feces palpable in colon, well-hydrated Gallop rhythm not auscultable today CWCT 1088 07/20/16 10:05 no change passed normal appearing stool eating wet food 7/19/16 No app this morning Pt 8% dehydrated today – decreased skin turgor Needs further diagnostics Increased LRS to 150 ml SQ BID for next 48 hours 1088 07/18/16 11:28 not icteric (tbili normal) chemistry unremarkable–needs t4 though well hydrated today open diagnosis suspect underlying disease guarded long term prognosis 07/17/16 18:44 vet check for straining to defecate and constipation. no feces seen in litter box, underweight body condition, dehydrated, geriatric. no blood results – techs unable to draw blood. moved to medical. slightly icteric mm’s (pinnae and 3rd eyelids), severe dehydration, not eating any food offered, intermittent gallop rhythm, emaciated body condition, does not feel constipated on abdominal palpation, unable to palpate a urinary bladder, but urine was present on bedding, A: geriatric underweight body condition, muscle atrophy icterus (mild) dehydrated not eating anemic – non-regenerative (likely due to chronic disease/neoplasia) P: move to medical LRS 100 ml SQ BID mirtazapine 3.75 mg PO SID x 2 days recommend NH placement for further diagnostics and supportive care prognosis: fair to poor depending on underlying disease — 7/16/16 S-small fecal present in litter box O-QAR, pink mm EENT-CAU, COU, no nasal d/c P-bring to medical for blooddraw

Reaction to touch: Kitty accepts the stroke, leans into the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: She calmly jumps back into the cage. Placement determination: Average Kitty interacts with the observer, appreciates attention, is easy to handle and tolerates all petting.
