Medical Summary DVM Intake Exam Estimated age: 6 years Microchip noted on Intake? History : was found in a fight with a raccoon Subjective: sedated for exam Observed Behavior – sitting quietly on table prior to adminstration of sedative Evidence of Cruelty seen – none Evidence of Trauma seen – bite wound to left thoracic limb Objective T = P = 180 R = 20 BCS= 4.5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: wnl PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, multiple scars on laterally on both thoracic limbs. Bite wound on top of left front over metacarpals, significant swelling from digits to just above carpus. Neurological-sedated but seemed mentally appropriate prior to sedation Rectal: not indicated Assement: Quarantine for possible rabies exposure, treat for bite wound Plan: 1. sedated with 0.11 mls of 100 mg/ml telazol IM 2.clip/clean bite wound 3.Radiographs of front limbs as bite wound is directly over a joint and he is already sedated for exam. The lateral film showed no obvious fracture but significant soft tissue swelling. The AP films were over exposed (attempted twice), could not read them 4. 0.5 mls of 80 mg/ml convenia given SQ 5.fvrcp,implant microchip Plan Prognosis: Guarded, rabies suspect SURGERY: Temporary waiver due to possible rabies exposure
