Observed Behavior – easily handled, no signs of aggression

P = 120 R = 20 BCS 5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: pink mm, deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male in tact
MSI: Ambulatory x 4, mutifocal alopecic lesions and crusting through face and trunk of body
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Ringworm
Plan: 1. Wood’s lamp : Positive
2. Lime Sulfer dip: Dip twice weekly until otherwise directed
3. Itraconazole: 0.25 ml by mouth once daily for 21 days.
Prognosis: good
SURGERY: Temporary waiver due to underage
LVT Exam 9/16/17:
Microchip: negative, did not place due to dermatitis over back
Sex: intact male
Age: appx 5-6w
Mentation: BARH
Eyes: clear
Ears: earmites (no ivermectin due to underweight, pinnal pedal and thick “grainy” d/c)
Nose: no d/c
Teeth: deciduous
If abnormal BCS: WNL 5/9
Skin: crusted lesions with hairloss throughout, apearance of ringworm with Woods+, DTM pulled, high suspect ringworm
Hair Coat: see above
Declawed: no
Any injuries: no
Behavior: allowed all handling, friendly and playful
Medication: N/A, preventatives given
