No History : Stray Subjective: Observed Behavior – QAR Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = wnl R = wnl BCS 7/9 EENT: Nuclear sclerosis, aniscoria (L>R), sunken eyes, drooling from right side of mouth, mild crusted nasal discharge Oral Exam: Moderate to severe dental tartar/gingivitis PLN: Significant enlargement (>3cm) of right submandibular lymphnode vs mass in that region, no other peripheral lymphadenopathy H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Spayed MSI: Ambulatory x 4, edema of right front limb (distal to mass), skin free of parasites, no masses noted, unkempt hair coat with discharge at right front limb CNS: mentation appropriate – no signs of neurologic abnormalities Radiographs: soft tissue opacity over 4th rib and between ribs 6-7, as well as just caudal to diaphragm; retroperitoneal and falciform fat opacity in abdomen Assessment: Rule out neoplastic mass (lymphoma vs adenocarcinoma vs other) with possible metastasis to lungs; geriatric, no interest in food, drinking water Plan: Continue to monitor. Consider EHR if condition declines Prognosis: Guarded
