Objective
BAR-H, MMs pink and moist, BCS 5/9
EENT: Mild mucoid ocular discharge, conjunctival erythema. Mild black granular discharge AU. Mild crusty dark nasal discharge.
Oral Exam: Clean adult teeth.
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, not painful
U/G: Female, prominent nipples, no SS, no vulvar discharge
MSI: Polydactyl front feet. Mild flea dirt. Ambulatory x 4, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment:
1. URI/conjunctivitis
2. Polydactyl
3. Suspected ear mites
Prognosis: Good
Plan:
1. Tresaderm AU BID x10 days
2. Clean ears q3 days x4 cleanings
3. Keep in isolation
4. Fortiflora SID x14 days
5. Vitamin B 0.25 ml SQ once
