Subjective: QAR, ~8-10% dehydrated. Has severe purulent d/c on face and diarrhea on hindend. Feels cool to the touch. Is alert enough to be resistant to exam.
Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures
Evidence of Cruelty seen – no

EENT: Eyes have sever purulent d/c, ears clean, severe purulent nasal discharge noted
Oral Exam: mixed kitten and adult dentition, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic with audible resp congestion
ABD: Non painful, no masses palpated
U/G: MI
MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment:
Underweight
Dehydrated
URI with conjunctivitis
Plan: Continue to monitor while at BACC
Parvocite-negative
Start SQ LRS 20ml/kg BID x5d until 1/8 (first dose given)
Start nutrical on gums BID x5d until 1/8 (first dose given)
Start doxycycline 10mg/kg PO SID x10d until 1/13 (first dose given)
Start terramycin OU BID x10d until 1/13 (first dose given)
Recheck daily
Heat support
Prognosis: Poor
