Medical Behavior Evaluation BLUE
Medical Summary 10/11/16 Microchip scan neg. O: BARH. Fearful and hiding in back of carrier, allowed all handling. ORAL: All decid. teeth erupted. EENT: Mod. congestion, mucoid nasal discharge. Severe chemosis, ocular crusting and mucopurulent discharge. H/L: Lungs clear. ABD/UG: Palpation WNL. Intact male. INTEG: full, sl. unkempt haircoat. A: Severe conjunctivitis, URI. Age est 2 months. P: Cleaned OU with warm water, applied Terramycin. Gave Amoxi-Clav 45.7 mg/ml 0.4 ml PO. Offered food – eating well. Continue Terramycin ointment OU BID x 10 days. Continue Amoxi-Clav. 0.4 ml PO BID x 10days. Good prognosis.
