Observed Behavior – QARH. Allows all handling. Excellent appetite
Evidence of Cruelty seen – No
Evidence of Trauma seen – No

P = WNL R = WNL BCS 3/9
EENT: Conjunctivitis and corneal rupture OU, ears clean, no nasal discharge noted
Oral Exam: Teeth clean, pink mm
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, mild flea dirt
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underage, underweight, thin BCS, corneal rupture/conjunctivitis
Plan: Cleaned eyes, applied erythromycin, Rec augmentin 0.2ml PO q12 x 10 days, erythromycin OU q12 x 10 days
Prognosis: Good, may need bilateral enucleation
SURGERY: Temporarily waived from surgery due to underage, conjunctivitis
