Subjective:
Observed Behavior -QARH. Tense in carrier. Allowed to be taken out of carrier, urinated large amount on table, fled exam table, very nervous, exam done in net
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
P = WNL R = WNL BCS 5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: Pink mm, teeth clean
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female, lactating
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Nursing queen, lactating
Plan: Continue to monitor while at BACC.
Prognosis: Good

Subjective: BAR, hydrated
Observed Behavior – a little shy, timid, allows exam and treatment with minimal restraint
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
P = 120 R = 20 BCS: 5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: missing a few lower incisors, good dentition otherwise.
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: female intact, no mgts or dc, lactating/nursing
MSI: Ambulatory x 4, 1+ fleas, no masses noted, healthy hair coat.
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment:

Plan:
continue to monitor at BACC
Prognosis: excellent
SURGERY: temporary waiver due to lactation/nursing kittens.
6 kittens – 3 week olds: Montgomery, Magnolia, Amelia, Eloise, Dale, Johnny A1126082, 83, 85, 86, 87, 88: (SAFE 9/22/17)
INTAKE INFO: 2 moms and 6 kittens – finder has been caring for for the past 2 years. She stated that the cats stay in her yard in a shelter that she built for them. When the mothers and the kittens arrived they were all together in the same carrier. owner stated that she was not sure which kittens belong to which mom and both of the mothers take turns feeding all of the kittens.
**** PLEASE NOTE **** KITTENS under the age of 8 weeks or weighing 2 pounds or less CAN ONLY be PULLED by a RESCUE!!!! IF INTERESTED, PLEASE contact a RESCUE ASAP!!!
