Medical Summary DVM Intake Exam Estimated age: 4 months based on dentition Microchip noted on Intake? neg on LVT intake History : brought in by good sam Subjective: BAR- H, alert and timid but able to handle for exam Objective BCS: 5/9 EENT: Eyes clear, black debris AU, no nasal discharge noted Oral Exam: all kitten dentition present. healthy gums and teeth, no oral masses PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact, both testicle descended MSI: Ambulatory x 4, live flea seen, healthy skin and hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: 4mo MI DSH 1. presumptive ear mites Plan 1. ear mites: clean ears and start ear mite treatment RX: tresaderm AU BID x 10days Prognosis: excellent SURGERY: Okay for surgery
