Medical Summary DVM Intake Exam Estimated age: 8 weeks (all deciduous teeth are in) Microchip noted on Intake? no History : A group of teenagers found kitten as stray running down a street in Queens Subjective: Kitten is BAR, meowing loudly at front of cage, appears to be eating some wet food Observed Behavior – Readily approaches veterinarian. Little restraint required for examination, kitten is active and moving around, but does not struggle or fight restraint. Allows all body parts to be touched without signs of discomfort. No “warm up” time needed – immediately social and interactive, purring, loves chin rubs. Not easily startled. Curious, interested in exploring environment, but easily focused back on people. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 150 R = purr BCS 3-4/9 EENT: Eyes clear but third eyelids protruding slightly OU, ears have large amount dark flaky/sandy debris, no nasal discharge noted Oral Exam: all deciduous teeth are in; no oral lesions seen PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl; unable to palpate both testicles, but at least one feels descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, coat appears healthy – observed grooming CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Underweight (by ~0.5lbs), Ear Mites Plan: Cleaned AU and applied Revolution for Kittens <5lbs to treat for ear mites. Clean ears prn +/- add Tresaderm if signs of secondary Otitis externa observed. Recommend foster placement until 2+lbs (and check for both testicles descended) or rescue placement. Prognosis: Good SURGERY: Temporary waiver due to underweight
