EENT: Conjunctivitis with mucoid discharge OU, ears clean, no nasal discharge noted
Oral Exam: Teeth erupting appropriately, pink mm, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated, distended
U/G: male
MSI: Ambulatory x 4, flea dirt, no masses noted, dirty hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underage, conjunctivitis, intestinal parasites, fleas
Plan: Rec erythromycin OU q12 x 10 days, clavamox 0.15ml PO q12 x 10 days
Prognosis: Excellent
SURGERY:
Temporary waiver due to underage
