DVM Intake Exam 8/5/17 Estimated age: ~3yrs Microchip noted on Intake? Negative History : Owner surrender Subjective: BARH Observed Behavior: tense and hisses/tries to scratch when approached. Used net to do exam and medical procedures Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = eupneic BCS 6/9 EENT: Eyes clear, ears clean, left ear crumpled pinna but no hematoma, no nasal discharge noted Oral Exam: Teeth clean, minimal dental tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, sniffing ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, no ectoparasites noted, no masses noted, healthy haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy Left ear crumbled from chronic condition, no hematoma and does not seem to be bothering him Plan: Continue to monitor while at BACC Needs behavioral eval Prognosis: Excellent SURGERY: neutered
