Medical Summary DVM Intake Exam Estimated age: ~ 10 wks Microchip noted on Intake? negative History : stray, injured Subjective: BARH, eating/drinking ok, playing in cage Observed Behavior – freindly, allowed handling and PE Evidence of Cruelty seen – no Evidence of Trauma seen – yes, non-weight bearing lameness at front left leg Objective P = wnl R = wnl, purring BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male, palpate both testes MSI: Ambulatory x 3, non-weight bearing lameness at front left leg, mild soft tissue swelling around elbow, tender to touch but seems comfortable; skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: bright alert kitten with non-weight bearing lameness to front left limb Plan: cont. pain medication, cage rest and get radiographs Rads: left mid-humeral fracture Prognosis: fair/good – will need ortho/surgical follow-up for fracture stabilization or amputation SURGERY: recommend temporary medical waiver from neuter surgery
