SAFE 7/11/18 HONEY is a neutered senior boy who is 13 yrs old. His owner brought him in to the shelter and said that Honey pulls out his own nails. Honey’s paws were bloody and he had some swelling on his right paw and also seemed to have a soft tissue mass there. This kitty needs someone who will get him follow up vet care to see why Honey does this. Please help him today.

History : reported that “Honey attempts to rip out his own nails…does this behavior about once or twice a month…came in today with bloody paws and the owner stated that he was taken to a vet 2 years ago…latches on to her furniture or fabric, and pulls out hard and uses his mouth to take out nails;” ventral haircoat covered in dried blood with some damage to front nails but none requiring soaking in novalsan or cutting, no embedded nails noted, overweight, mats and dander across back (combed out)

EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: teeth in good cond
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Neutered male
MSI: Right forelimb – 3rd digit – Marked bleeding, swelling, pain.
Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: externally normal
Assessment: traumatic injury vs self induced trauma vs mass effect
Right-fore 3rd digit soft tissue swelling/bleeding/pain r/o SOFT TISSUE MASS
Plan – Complete intake treatments
– Clavamox 62.5 mg (1ml) suspension BID for 14 days
– Onsior 2mg/kg SQ SID for 3 days
– Simbadol 0.2mg/kg SQ SID for 3 days
– Bandage right fore with a 3 layer bandage
Make arrangements with sx team to sedate and clip, explore, clean affected area
