EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: not examined
H/L: Grade 3/6 heart murmur auscultated, clear lung sounds bilaterally
ABD: Non painful, no masses palpated
U/G: Male (NEUTERED)
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment; Grade 3/6 heart murmur — mildly think body condition — hyperthyroid suspect
Plan; CBC/CHEM with T4 recommended. No surgery today due to finding that he is already neutered. Recommend further workup with local veterinarian regarding heart murmur.
Prognosis: Fair

urination and appetite check-
no urine, and uneaten food observed in bowl. as per ACS, cat is not eating food and no urine found in litter. vet has been requested to check this cat.

Hollywood is a 4 year old gray and white neutered cat who was surrendered by his owner due to behavior concerns. Owner stated that when around strangers Hollywood is friendly and outgoing. When playing with adults he is somewhat rough. Hollywood has spent time in the home around children and he is usually tolerant. When he does not want to be bothered he will swat at children and scratch them. Hollywood has not spent time in the home around other cats so it is unknown how h will react. Hollywood has not spent time in the home around dogs so it is unknown how h will react. Owner stated that Hollywood has never been bathed brushed nor had his nails trimmed so it is unknown how he will react. He isn’t bothered if he is held, put in a carrier or disturbed while he sleeps. Hollywood has bitten a person before but has never bit another animal. No reported medical concerns. Owner described Hollywood as friendly, playful and confident. When at home he likes to be in the same room as his owner. He likes to play with balls and stuffed mice. He has been kept indoors only and eats both wet and dry food. He is used to an uncovered litter box with clumping litter. Hollywood scratches on a post made out of card board. During intake Hollywood allowed the counselor to pick him up several times, take his picture and collar him with no problems.

Standing in back of kennel, ears forward. He leans in for petting with his shoulders and steps forward, tail raised to allow further petting. Doing well after first day! Sprayed feliway.
12/14/17
Lying in litterbox with a relaxed body, ears forward. Makes eye contact with soft eyes. He allows petting along his head and body while leaning in and standing up, back arched and tail raised. He steps forward with continued petting, tail beginning to quiver as it becomes pilo-erect along the base. Stands up on hind legs and puts front paws on my shoulders. Very sweet and playful, doing great! Shows signs of overstimulation.

Lying on top of kuranda bed with a relaxed body, ears forward. Makes eye contact when spoken to and leans in for petting once touched. He jumps off of his bed to allow further petting, tail becoming pilo-erect at the base as it waves back and forth. Calm – allows all petting for duration of session while purring softly and making eyes contact with soft eyes. Not interested in treats today. Doing great!

Lying on top of kuranda bed, body relaxed and ears forward. He makes eye contact when spoken to and jumps off of his bed to greet my hand with a head-butt. Allows petting along head and body while leaning in, tail raised. Tail becomes pilo-erect along the base but he continues to allow petting for the remainder of the session. Places front paws on my shoulders and head-butts my cheek. Affectionate and playful – did great today!

Lying on top of kuranda bed, body soft and ears forward. Reaches out towards me with soft paws. He jumps down as I open the kennel door and greets me with a head-butt. Allows petting along head and body while leaning in, tail raised and quivering. Stands up and places his front paws on my shoulders. Friendly and social, allows all petting for duration of session. Doing great!

Tail becomes pilo-erect at the base but he offers no further reaction and allows all petting for the remainder of the session. Purrs and places his front paws on my shoulder so he can head-butt my chin. Sweet and affectionate, doing great! Not too interested in treats.

He remains in front of his kennel and allows petting along his head and body while standing up and leaning in, tail raised and quivering. Very sweet, remains calm and allows all petting for remainder of session. Doing great!
